Limited financial assistance is available to eligible families. To apply for financial assistance, complete and return the Financial Information Form.
For campers who have not attended our 2012 Respite Programs or Summer L.I.F.E. Camps:
- Complete and return the Respite Registration Form.
- Complete and return the New Camper Intake Form.
- To apply for financial assistance, complete and return the Financial Information Form.
- Once the forms have been received and camper is accepted into the program an invoice will be issued. Payment in full is required to secure your child’s spot in the program.
- Complete Emergency Information and Medication Card.
- Complete Medication Dosage Form (if your child will require medication during the program).
